The disaster is the deadliest involving a train carrying crude oil.The Transportation Safety Board of Canada (TSB) investigated the accident and concluded that it was caused by human error, specifically, a failure to apply several safety procedures related to securement of trains in Nantes. The TSB also found MMA's safety management system deficient and noted that Transport Canada had not effectively regulated this industry segment.On 15 July 2013, Quebec police arrested three individuals: Tom Harding (the railway's operations manager), Jean Demaitre (the traffic controller) and Richard Labrie for criminal negligence causing death; they were later released on bail with conditions pending further investigation.[3] On 18 December 2014 all charges against them were dropped due to lack of evidence.[4][5]In January 2015 Montreal Maine & Atlantic Railway filed for bankruptcy protection in both Canadian Federal Court[6] as well as United States Bankruptcy court,[7][8][9].
